Ask for access to the KTH form service

KTH's form service at www.kth.se/form enables the creation of simple web forms for the collection of standardized response data.

To access the service, fill out the form at the bottom of the page. But first read through the text below.

Users of the service (editors) need to understand:

  • Only employees at KTH are granted access to the service
     
  • The editor of the form is equivalent to the person responsible for personal data and is obliged to delete the submitted personal data as soon as the processing of them is executed according to the purpose of the form
     
  • When collecting personal data, the General Data Protection Regulation (GDPR) and KTH's obligations towards this must be taken into account. Read more about GDPR at KTH  and what is important to keep in mind when it comes to handling personal data through the form service .
     
  • When collecting sensitive personal data, KTH's Data Protection Officer must always be notified: dataskydd@kth.se .
     
  • The service may not be used for the collection of classified information
     
  • Replies received stays in the application and are not further registered according to document management regulations at KTH. It is up to the user of the service to secure such handling himself.
     
  • Editors undertake to monitor incoming responses. As an public-sector authority, KTH is obliged to handle all information received via our digital channels. It is therefore important that you as an editor ensure routines for monitoring created web forms and that forms that are not covered by such control are closed down.
     
  • Replies received are stored in the form service, even if they are only handled by email, and it is up to the editor to delete answers that should no longer be registered
     
  • Incoming responses are also logged in underlying systems, but these logs are cleared periodically. Thus, responses that are deleted by the editor are only accessible via log files for a limited time.
     
  • The form service is a web application and technology can fail. Disruptions on the client side or in KTH's operating environment can mean that something goes wrong when a respondent answers a form. Read more about what you as an editor can do to handle this .
